However, the long-term effects and potential consequences of this disease are unavoidable. Diabetics can regain control of their health by learning self-management skills such as blood glucose-ketone monitoring, recording keeping and insulin preparation and administration and meal planning.
Care Lync Coordinators help Medicaid-eligible diabetic adults navigate the Attendant Care Waiver program and additional resources through the Independence Waiver. Our expert team helps Participants access the community-based services available to them. From health maintenance activities to personal emergency response services, we help waiver Participants get the help they need in order to feel better, stay more active and slow the progression of the disease.

Care Lync provides service coordination for adults diagnosed with Physical, Intellectual and Developmental disabilities who are enrolled in the Department of Human Services Medicaid Waiver Programs.
