Meet Your Dialysis and Chronic Kidney Disease Care Coordinator Services Team.
Chronic kidney failure is a slow and progressive disease. Since the other kidney can carry out normal functions if the other one stops working, many CKD patients don’t recognize the warning signs until the disease is well advanced.
There are, however, some therapies that can help control the symptoms, reduce the risk of complications and, in some instances, slow the progression of the disease. The goal is providing services that can help promote quality of life while maximizing bodily function.
Care Lync Coordinators help Medicaid-eligible adults with chronic kidney disease navigate the Attendant Care Waiver program and additional resources through the Independence Waiver. Our expert team helps Participants access the community-based services available to them. From daily living assistance to home health services, we help waiver Participants get the help they need in order to return to a life of independence, self-worth, and dignity.
