Helping People with Hearing and Vision Loss.
Service Coordination You Can Rely On.
There are many conditions that can lead to hearing and vision loss. While hearing loss may be gradual, vision loss is often sudden – or vice versa. Managing the issues of both often requires addressing the underlying cause.
Whether hereditary or a result of getting older, hearing and vision loss has a significant impact on a person’s development and ability to communicate. Treating hearing loss requires a team effort with the aim to improve the Participant’s quality of life and increase independence.
Care Lync Coordinators help Medicaid-eligible adults with vision and/or hearing loss navigate the Attendant Care Waiver program and additional resources through the Independence Waiver. Our expert team helps Participants access the community-based services available to them. From daily living assistance to home adaptation, we help waiver Participants get the help they need in order to return to a life of independence, self-worth and dignity.
