Improving Morbid Obesity Management.
Service Coordination Care to Help You Achieve Your Weight Goals.
There is no single or simple solution when it comes to treating morbid obesity, which is one of the most pervasive diseases in the United States. Defined as an excess of adipose tissue – or fat – this condition may lead to other more serious chronic diseases such as diabetes and numerous cancers.
While weight loss is based on lifestyle changes, this is difficult to achieve and even harder to maintain. The goal is to help people with morbid obesity stay on track towards a longer and healthier life.
Care Lync Coordinators help Medicaid-eligible adults with morbid obesity navigate the Attendant Care Waiver program and additional resources through the Independence Waiver. Our expert team helps Participants access the community-based services available to them. From daily living assistance to non-medical transportation, we help waiver Participants get the care they need in order to return to a life of independence, self-worth, and dignity.
Care Lync Service Coordinators can help make life more manageable and accessible for those with morbid obesity by helping with the following:
Waiver assistance: Care Lync Coordinators help Participants develop a service plan that best suits their needs and goals.
Service guidance: Our will seamlessly coordinate and monitor the provision of service providers, ensuring the health and welfare of each patient.
Community Integration: By helping patients get access to the services provided to them under their waiver, we hope to help them adjust to a full, independent life.
