  • Coordinate services needed by each participant, across agency lines
  • Creates Individual Service Plans (ISP) unique to each participants needs
  • Serve as the single point of contact in helping participants and their families to obtain the support and services they need
  • Monitors the health and welfare of the participant on a regular basis
  • Facilitates referrals to additional agencies, both waiver and non-waiver, if appropriate
  • Identifies available providers
  • Monitors the delivery of services
  • Evaluates and assesses participant Individual Service Plans (ISP)
  • Advocates on behalf of the participant for additional support and services as needed
  • Assists in identifying and sustaining a participant’s natural support system including family, friends and associates
  • Protects the participant’s rights and needs
  • Maintains documentation of the participant’s eligibility for waiver services

Care Lync provides service coordination to adults diagnosed with physical, intellectual and developmental disabilities who are enrolled in the Department of Human Services Medicaid Waiver programs. We assist the individual with locating, coordinating and monitoring home and community based services that enables the participant to live an independent and healthy lifestyle within their home and community.
