Care Lync is an approved provider within the Department of Human Services Home and Community Based Waiver Services Department. Our team of highly specialized coordination experts work with service care providers on behalf of Medicaid-eligible waiver Participants to help minimize the obstacles and maximize efficiencies in service delivery.

Care Lync provides service coordination for adults diagnosed with Physical, Intellectual and Developmental disabilities who are enrolled in the Department of Human Services Medicaid Waiver Programs.
